Leukaemia is commoner in males. 1 The diagnosis of acute leukaemia is suggested by history and physical examination, and is confirmed by blood counts, and the examination of a blood film and bone marrow aspirate. In patients with a previous myelodysplastic syndrome, anaemia, leucopoenia, and/or thrombocytopenia may have appeared several months or years earlier. The use of morphology, cytochemistry, cytogenetic and molecular analysis allows differentiation into subtypes of AML or ALL. The French–American–British (FAB) group divided the AML into eight subtypes (M0–M7) and the ALL into three subtypes (L1–L3).

Recovery of bone marrow and blood production is achieved by returning the frozen stem cells to the patient after the chemotherapy has been cleared from the body. 5% of patients undergoing this procedure die of infection or bleeding. Allogeneic transplant is rarely used in the management of lymphoma. Treatment is carried out in specialist transplant centres and the patients are young. The aim of allogeneic transplant is to cure the disease by administering high doses of chemotherapy and using the donor immune system to destroy the patient’s lymphoma.

Oral treatments can be used for all these infections when they are confined to the skin. These treatments include acyclovir, valacyclovir, and famcyclovir. The last two drugs have a longer half-life and are better absorbed than acyclovir. The highest dosages of these antiviral agents are used for the treatment of any VZV or a disseminated HSV infection, while lower dosages can be used for localized HSV infections. Patients who have organ damage, or who are severely affected, may be treated by intravenous therapy initially and be switched to oral treatment once the infection is under control.
